Analysis of 2014 and 2015 HBHC Postpartum Screening Tools and IDA data was undertaken to determine more effective and efficient ways of identifying high risk families in Peel. The results of the analysis will inform managers of the local HBHC program of possible process changes to improve identification of families which truly require the home visiting program.
