Abstract:
Consenting women are referred to the HBHC program by health care providers during three stages: prenatal, postpartum, and early childhood. Originally, women were referred using three different tools: the Larson screen used prenatally; the Parkyn screen during the first six weeks postpartum; and the HBHC External Referral Form thereafter. Following a period of development, the Ministry of Children and Youth Services (MCYS) introduced, in 2012/2013, a single new HBHC Screening Tool to be used for all referrals, regardless of the entry stage (See Appendix 1). The goal of the tool was to provide sufficient information to move families directly from screening to In-Depth Assessment (IDA) or discharge.
